Pathways to a Healthy Community - An
Indicators & Evaluation Toolkit - This guide
by the Ontario
Healthy Communities Coalition is an annotated bibliography of evaluation
tools for community health organizations. The guide itself
doesn’t provide evaluation tools, but instead suggests
tools from other sources that you might find useful. It provides
complete information on each evaluation tool, where to get
it, who to contact, and the cost, if any.
Strategic Review: Community Health
Centre Program - This evaluation
from the Ontario Ministry of Health reports on the effectiveness of
Community Health Centres in Ontario, and makes recommendations
to strengthen their usefulness and accessibility. The report
has many positive things to say about Ontario’s CHC
program, and lays out plans for expanding the influence of
CHCs in the health sector.
